STEP 1) Service Request Number:
In order to qualify, you must 1st call for your "Service Request Number" which will then be added to your initial application form.
STEP 2) Fill Out the Application:
Before you can submit your information for rebates, you are required to fill out the 3P Program Rebate - Application Form. Below we've provided links to "SAMPLE" forms in order to help you with this process, along with the actual application form.
STEP 3) Obtain 3-Separate Quotes:
This is a VERY important step and must be completed with accuracy. The Hamilton Water Department has strict guidelines on how this can be obtained. Please see the below checklist for requirements needed while submitting your application.
